Annual memberships for pediatrics
- 1 "well exam" per year, up to 1 hour
- 1 online "sick visit" per month, up to 30 min
- 2 online maintenance visits per year
- 1 in-home sick visit/month
- Telephone/chat consult 24/7
- $1,200 for the year (first child)
- Each additional sibling signed up for membership receive a 10% discount, $1,080 per year
- One time new patient evaluation/enrollment
(Additional charges may apply for home visits, such as after hours, long distance, additional well checks for children under 3 years of age, lab tests) .
Pediatric Wellness Exams
What to Expect
Wellness visits are performed in the clients home or at school. Visits typically last up to 1 hour. Depending on the age of the client there may be developmental, vision, hearing or other screenings.
Wellness Visits (newborn, 2wks, 1mo, 2mo, 4mo, 6mo, 9mo, 1yr, 15mo, 18mo, 2yr, 2.5 yr, 3-21yr)
Wellness consultations (nutrition, BMI, labs, exercise plan) follow-up every 1-2 months
Sports Physicals/forms
Review annual visit/school forms
Review Immunizations (We DO NOT administer vaccines at this time)
Lab orders

What to Expect
"Sick" visits may be performed at the clients home, school, or if appropriate, via telemedicine. New clients must be registered and have initial consultation before being seen. This can be done at the time of the first visit.

Urinary Tract Infections (UTI’s)*

*Antibiotics will not be called in for "possible" strep throat or urinary tract infections (UTI's) without lab confirmation; or ear infections without visual exam.
